As a Care Coordinator at Horizon, you will…
  • Meet members in their homes or in the community.
  • Connect members to resources and services in the community.
  • Contact providers to coordinate the members’ care.
  • Complete all appropriate documentation of the members interactions.

What makes you a great candidate?
We can’t wait to learn more about you! Here are a few specifics of what you’ll need for the job:
  • Bachelor’s Degree in a Human Services related field; OR
  • Credentialed Alcoholism and Substance Abuse Counselor (CASAC).
  • Experience providing care coordination services or working with mental health, substance abuse or medical conditions in another capacity is very helpful but not required.
  • Excellent verbal and written communication skills.
  • Ability to work independently while also being part of a large team.
  • Organizational, time management, and problem-solving skills.
  • A clean, valid, NYS driver’s license, three years of driving experience, and reliable transportation to travel locally on a regular basis.
Location:
1000 Young Street, Tonawanda, New York
Hours: 
Monday through Friday - no weekend hours in this position! This position has a schedule from 8:00 AM - 5:00 PM but we are flexible and want to work with your schedule as well!
