Behavioral Health Coordinator with sign on bonus

Working within a multi-disciplinary team, provides comprehensive case management services to improve mental health, and in conjunction with the primary care provider, improve medical health to adults with serious and persistent mental illness.
Responsibilities:
1. Completes psychosocial assessments, skilled at identification of needs and resources, and development of individual treatment and crisis plans.
2. Coordinates and facilitates access to preventative and health promotion services, mental health and substance abuse disorder treatment, and care coordination services.
3. Transports clients for clinically appropriate purposes.
4. Maintains BHH census at targeted level.
5. Performs complete and accurate documentation within the electronic medical record according to system policies.
6. Maintains and exhibits a current knowledge of profession and participates in continuing education.
7. Maintains a safe environment complying with NLH policies and procedures; reports and directly addresses environmental hazards and violations of patient safety policy and/or protocol when involved or observed.
8. May perform other duties upon request.
