Care Coordinator

Sunrise Services, INC. is seeking a Care Coordinator for our Health Homes Program! Break into the rewarding field of human services! No prior experience required w/ bachelors in applicable field.

About the Program:
Health Homes is an outreach program which engages clients (including pediatrics) in the community with health and social services to best meet their needs. Health Home (HH) Services are intensive services that coordinate care across several domains. The purpose is to coordinate clinical and social service expertise for high risk clients with complex chronic conditions, mental health challenges, substance use disorder issues, and/or long term service needs and supports. Primary duties of a Care Coordinators include comprehensive care management, care coordination and health promotion, comprehensive transitional care, individual and family support, and referral to community and social support services.
 
Duties & Responsibilities:
  • Assess clients’ presenting problems, formulate individualized health action plans, assess, monitor and document clients’ response to interventions. Evaluate effectiveness and revise plans regularly.
  • Link clients to health and social resources based on individual needs and preferences
  • Accept, respond to and process referrals received from Lead Entities
  • Engage in outreach activities to contact clients, and participate in face to face visits with clients in a location of the clients choosing
  • Foster communication between providers of care including primary care providers, medical specialists, personal care providers, and others
  • Make arrangements for transitional care from inpatient to other settings if needed
  • Report all suspected incidents of abuse or neglect of vulnerable adults and children as required as a mandated reporter
  • Maintain accurate and timely records. Track, monitor and document items in accordance with policies, procedures and regulations.
  • Support the achievement of individualized health goals designed to attain recovery, improve functional or health status or prevent slow declines in functioning
  • Assess clients’ presenting problems, formulate individualized health action plans, assess, monitor and document clients’ response to interventions. Evaluate effectiveness and revise plans regularly.
  • Perform other duties as assigned.

Requirements: (Any Equivalent Combination of Knowledge, Skills, Abilities, Education, and Experience)
 
Education: (Must meet one of the following criteria, education may be substituted for applicable experience in human services)
  • Bachelors level degree in social work, psychology, social services, human services, behavioral sciences or a related field
  • Certified Medical Assistant (CMA) with an Associate’s level degree
  • Significant experience related to the mental health field and no degree
Experience:
  • At least two years of experience in the mental health field (preferred)
Licensure/Certifications:
  • Washington state driver’s license
  • First Aid and CPR certification
  • Licensure related to qualifications listed above
Other Considerations:
  • DSHS background clearance including fingerprint
  • Acceptable driving record
  • $50,000 automobile liability insurance required
  • Strong verbal and written communication skills
  • Skilled with Microsoft Office Suite
  • Ability to lift approximately 20-30 pounds and physically assist unsteady clients
